Family Scholarship Program from Solvay Pharmaceuticals
The purpose is to provide financial assistance toward higher education for students with Cystic Fibrosis (CF) who demonstrate high
academic credentials, the ability to serve as a role model for others with CF, and the need for financial assistance.
WHO IS ELIGIBLE?
All U.S. citizens with CF who will begin or continue higher education in the fall of 2007. Applications must be postmarked by the
deadline of June 29, 2007. Applications postmarked after June 29, 2007, will be ineligible
WHAT IS THE SCHOLARSHIP AWARD?
Upon meeting eligibility requirements, forty (40) applicants will be awarded a $2,000 scholarship for each of two years of
matriculated study for his or her current degree program. Scholarship recipients are eligible to apply for additional scholarships at the end of each two-year award period.
In addition, after submitting an original prescription from his or her physician, each recipient will receive a two-year supply of CREON MINIMICROSPHERES (Pancrelipase Delayed-Release Capsules, USP) Brand pancreatic enzymes, a two-year supply of SourceCF® Softgel vitamins (24 bottles of 60 count) and a two-year supply of Nestle® Carnation® Instant Breakfast Plus nutritional drinks (24 cases of 24 count). The original prescription and a copy of the state business license for the medical facility listed on the prescription must be
remitted during the first year of the scholarship award. Prescriptions received after the first year of the award will be null and void. Please see full prescribing information for CREON® MINIMICROSPHERES® enclosed.
HOW DO I APPLY FOR THE SCHOLARSHIP?
This application must be completed by the student, personal physician, parent(s)/guardian(s), and a school official. An original nacademic transcript, a copy of your 2006 Federal Income Tax Return, an original photograph of the student, a letter from a
school official describing the student’s academic accomplishments, a second recommendation from another individual not related to the applicant, the signed consent and verification, and a creative representation must be included. The creative representation must
address what you have learned from living with CF. It can be in the form of a photograph, painting, sculpture, VHS video, CD-Rom, DVD, poem, essay, etc. We welcome and encourage a wide variety of creative expressions. Only one complete application per student will be accepted. Incomplete applications will not be eligible.
HOW ARE THE RECIPIENTS SELECTED?
Recipients will be chosen by a selection committee (panel of judges). The Solvay Pharmaceuticals, Inc. selection criteria include academic achievement, creative representation, and financial need. The judges will base their decision on students’ academic records, references, creative representations, and financial resources. Recipients will be notified during August 2007. All decisions are final and materials submitted will not be returned. Solvay Pharmaceuticals, Inc. reserves the right to alter the terms of this program without notice.
